Under current public health orders, businesses are being asked to collect information to help public health officials track the transmission of COVID-19 and alert those who may have been exposed to keep Manitobans safe and reduce the spread as much as possible.
For businesses: all information on the form below must be collected from patrons if your business is a restaurant or licensed premises, theatre, museum, gallery, library, personal service business, concert hall, or fitness facility.
In the case of a restaurant or licensed premises, contact information must be collected from at least one person in each party. In the case of a theatre, museum, gallery, library, personal service business, concert hall or fitness facility, contact information must be collected from each person who attends the premises. If a business has a procedure in place for members to electronically record when they attend the facility, such a record can be used to satisfy the requirements of the orders.
The information must be securely retained for 21 days, after which it must be securely destroyed. Staff collecting the information should fill out the document themselves, should never ask patrons to fill it out, should never leave the document unattended and should store it securely after-hours. The information collected should only be used by Manitoba public health officials.
For the public: when you visit a restaurant or licensed premises, theatre, museum, gallery, library, personal service business, concert hall, or fitness facility, you must provide your contact information in order for Manitoba public health officials to alert you in the event you have been exposed to COVID-19 while visiting that facility. Your information will be securely kept by the business, will only be available to Manitoba public health officials, and will never be used for any purpose other than assisting with COVID-19 contact tracing.
