Replace your Health Card
If your Manitoba Health Card is lost, stolen, or damaged you can request a replacement below.
Before starting the online form:
Changes can only be made by the card holder, their Power of Attorney/Substitute Decision Maker/Committeeship holder.
Power of Attorney, Substitute Decision Makers and Committeeship holder documents must be on file with our office, if not please contact 204-786-7101 or 1-800-392-1207(toll-free) and we will provide information on the document process.
All forms must be signed in ink or with a verified digital signature prior to being submitted to Manitoba Health.
If you are not living at the address on your current Manitoba Health Card, use the change of address form instead of requesting a replacement card.
Option 1
To submit your request online you will need the following information:
Your Health Card Registration Number and Personal Health Identification Number (PHIN).
If you are ready to request your replacement Health Card, click the following link:
Replace your Manitoba Health CardA confirmation ID will be provided after submitting your request, please keep it for future reference.

If you do not have your Registration number and Personal Health Identification Number (PHIN) available please follow these two steps:
